I have encountered similar patients as you described. It's just how people are. Self centered. Some want to talk and be heard. They use the encounter with the doctor as their attachment piece. Loneliness. If we only had to care for this one person for an hour or so we would be ok. But the people who run clinics and hospitals don't do the work we actually do and just monitor metrics.

It's difficult work. Not always because of the technicalities but because we deal with people who have emotions and souls.

And too often I have seen doctors are so egoistic they believe themselves to be the best dr in the universe. And so often each one of us is left to fend for ourselves alone.

Why are there no doctor support groups? Doctors coming to share their difficult days and know they are not alone?

Toxic culture in medicine.

No rotation into preventive
Just some cancer screening u learn on the fly
Think really got do mph ?

Paed and o&g is 3 months rotation each nia ma.
Some GPS got mrcpch, some got mrcog
Psy ? Some programs don't even rotate thru it.

Mrcp is the best la.

Nurse take over role of GP.
AP GPN - advance practice GP nurse
Can prescribe flu meds
Push everyone to become specialist

APN can see urti, do referrals and manage simple chronic disease

Pay a Apn 5k
1 FP can pay for 5 apn

One polyclinic 15 fp can pay for 75 apn.
Maybe let 1 FP remain to tank court case
No more waiting time with 70 Apn

Someone give this smarty pants a beer ( not tiger one though) and send him to moh to work
