Not much difference
Both refer our.
Fam med refer out to other specialists at outpatient setting
Int med refer out to other specialists at inpatient setting
Both like to do a lot of workflow/quality improvement kind of stuff

So from here you can tell what kind of work is most highly valued,
Stuff that CANNOT refer to anyone.
Basically you're it. Which means you're the specialist in it.
There are some areas of medicine that are very niche. Some specialists also have no experience in that topic or treatment.
But it is still within the practice of medicine. If you're the GP providing the treatment then you're responsible for it. No referring out.
This is where the high value fees are found.
