Regarding #1 - they can submit a form to request for staff. They can ask and ask, over and over, but it doesn’t guarantee you will get. There’s nothing they can do because they are not in charge of manpower allocation or recruitment. What do you want me to do? Spend my own money to hire a part timer to help you ah?
Managers are just human shields with no real power.

For #2, I cannot speak for every manager. Some of us do go to clients place. I rotate among my portfolio.
Reasons for not wanting to go may include
a) not eligible to claim taxi after promoting to manager. Hence avoid unnecessary trips.
b) unable to teleport from east to west when you have multiple commitments
c) need to be in office to attend other meetings.
d) deliberately playing a good cop bad cop manager so your team will tell client it’s the manager insist on something so client empathises and supports them more.
e) expectation of juniors for managers to treat lunch or buy drinks when they come clients place.

#3 - depends on the big boss. Time cost is not a big deal for my department. My bosses are fine maintaining margins. Some new managers scared to approach big boss to ask to lower margins. It’s not true that margin is a manager KPI but some M1s have that misconception. Other bosses prioritise quality and service over budget. Just need to be reasonable.
Also some managers spoil market by hogging staff even when there’s not much to do just in case something comes in and they cannot get more people last minute.
