Just wondering, if I were to make an inter-ministry transfer at the end of the year, will I be appraised by my former ministry or the new ministry or will it be a default C? Thanks!

If you transfer before June, new Ministry appraises you - they will sought old Ministry's recommendation. After June (i.e. 1 July), old Ministry appraises you.
However, in both scenarios, the chance of getting C is higher than normal because there is a tendency to prioritize those who stay.

Then what about if resign from old ministry and join new ministry?
