Which mx grades have 2 ranges? What of someone hits the ceiling of even the higher range, will they be notified or will they just get a rude shock the following year when there is no increment?

Correct, under contract law, once you make a counteroffer (trying to negotiate for higher salary), the original offer is considered legally rejected. So HR is in the right if they decide to ghost you after that.

I think NS has a role to play in this. I know a male, fresh grad, 2nd upper, drawing 4.7k. 4.5k seems all right if female with 2 YOE.
