Is it very easy to be put on pip? What’s the process like? Is client’s input the deciding factor?

Few years ago, there was a guy in my team who was placed on PIP because he could not complete simple tasks independently and kept saying that he had blockers. Other teammates had to take over his unfinished tasks at the last minute. He was gone within 1 month of being placed in PIP.
