In one of the banks I worked for not to long ago, the entry position was called an Assistant Director. After 2 years, one become an Associate Director. If you got along well and didn't get fired along the way, you will become an MD in 10 years. Of course there was a complex setup of MDs as well. My boss (an MD) reported to another MD who in turn reported to yet another MD, 7 MD in the line. Then came COO and then executive COO and then the President and then the CEO. Almost 15 layers between the Assistant Director and the CEO.
