In USA they pay up to 37% tax, while top tax bracket in Singapore is just 22% tax.

Cost of living in Big cities of USA much higher.
Malpractice insurance and liability in USA also much higher.

In Singapore you just pay $100k for fine for failure to take proper consent, in USA you gotta pay a few millions for the emotional distress.

Being a doctor in Singapore is so much better, trust me. Although you may not think so but Once you adjust for purchasing power parity, taxes, medical liability, you will see the light.
