But if you put your supervisor/boss as your referee, doesn't that kind of mean that you are looking to resign?
It's all good if you eventually accept the new job offer and leave, but if you do choose to stay, it will be very awkward?
- why would a boss hand any important tasks for you to do if he know you are thinking of leaving?

There are different kinds of employment checks.
Some is just to ascertain information you submitted is true, some others try to find out more about your background.
In any case, most professional organizations (at least with regards to SG govt) will check with your current employer only after you left (assuming you are the one they hire).

Does one usually tell their current boss/supervisor that they are putting him down as a referee for their new employment company before actually doing so? I would imagine it's good courtesy to do so

Obviously if you don't want to let the game out of the bag, don't include a referee from your current workplace. Previous workplace/s is fine. If fresh grad, lecturers or TAs are fine. You can technically put friends, but not recommended since they cannot be expected to be objective.
