I know by now should probably be fixed.. I am just surprised they know our grades as early as last year since going through the many layers of approval probably take months

To be accurate, actually it's not fixed last year. It's that the department decided on the grade for you that may be moderated when benchmark at ministry and subsequently wog level.

Thankfully in most cases, grades don't change much that's why it appears fixed.

Will your supervisors have a separate chat with you if they were to give you a performance grade below C?

That might be quite unrealistic and unfair for the hiring agency. You can speak to your new RO/Director to see if this is possible. But you may risk giving a bad impression even before joining them.
