An analogy would be like a proposal. If a guy proposes to a girl, it is understood that both of them agreed to get married but the marriage certificate is not signed yet. So if the guy retracts on marriage, it would have detrimental effects on the girl and the guy would come across as being irresponsible.

but unlike a proposal, a job offer is based on a very limited knowledge of the other party (1-2 interviews, 3 tops in most cases), and it's generally a multiple-to-multiple relationship for both jobseeker and employer. So a proposal of a job offer is much less serious compare to proposal for marriage.

The buttomline is, unless it's in black and white (not even email), don't take it for granted that it's secured.

Do not wish to name names but I'm frustrated with the irresponsibility and lack of accountability of some organisations.

Likewise. I was given verbal offer a month ago and was told to wait a week for the contract. It's been a month already and they've promised to contact me by today (after the third call up I made to them last week).

Same situation. Given a verbal offer end Aug, now waiting for next follow up action.
